Click on either the "DOB certificate" or "Death certificate" option from the displayed list to proceed with the respective service. Click on the "Next" button on the Supervity instruction widget.

Click on the "Search" option to get the registration unit number, id, and year.

Select either the "Municipality" or "Panchayathi" option from the list, based on your requirement or preference. Click on the "Next" button on the Supervity instruction widget.

Enter your date of birth in the format DD/MM/YYYY, where DD represents the day, MM represents the month, and YYYY represents the year. Click on the "Next" button on the Supervity instruction widget.

Select your district from the available options. Click on the "Next" button on the Supervity instruction widget.

Select the registration unit ID, which refers to your birthplace or the place where the event for which you need the certificate occurred. Click on the "Next" button on the Supervity instruction widget.

Select your gender from the available options. Click on the "Next" button on the Supervity instruction widget.

In this process, we are providing mandatory fields to be filled. If you have any additional data for other fields, fill them in as well. Once all the required information is provided, click on the "Get Details" button. By clicking on the "Get Details" button, a list of records will be displayed. Select the record based on your parent's name and other relevant information. Note down the unit ID, year, and number associated with your selected record.
